Article II. NONEMERGENCY MEDICAL TRANSPORTATION |
§ 20-103. Records.
(a)
Licensees shall maintain records in accordance with this article and shall also maintain the following information:
(1)
Current personnel records that indicate date of employment and the qualifications of each employee.
(2)
A record for each vehicle containing:
a.
Vehicle registration;
b.
Record of each vehicle inspection;
c.
Copy of county permit; and
d.
Documentation of current insurance.
(3)
Each licensee shall keep a trip report recording the time, place and origin, name of passenger and destination. Each licensee shall retain and preserve all daily trip records for no less than three years. Such records shall be available for inspection by the BOCC and the state agency having jurisdiction, upon request, during normal working hours.
(b)
All records shall be indexed and filed for ready access.
